Where Do I Start MHMR Services?

Step One: For MHMR services, come into any of our Centers or contact us for a phone interview at
817-335-3022. 

Step Two: Prepare and gather documentation that is needed to complete your intake.

Required Documents:

• Picture ID
• Proof of Insurance (if applicable) 
• If you do not have insurance and need benefit assistance in filing for Medicaid, please also bring documentation for proof of income.

Step Three: In-person screening completed at any of our Centers.

Step Four: If you are eligible, an appointment will be offered at the Center. If you are not eligible, other community resources will be offered.
