MHMR ensures that patients’ rights are respected and met.

It is the policy of the MHMR that no qualified individual with a disability shall, by reason of such disability, be excluded from participation in or be denied the benefits of the services, programs, or activities of MHMR, or be subjected to discrimination, harassment, intimidation, or retaliation by MHMR.

ADA Complaints or Grievance
This ADA Grievance Procedure is established to meet the requirements of the Americans with Disabilities Act of 1990 (ADA). It may be used by anyone who wishes to file a complaint alleging discrimination on the basis of disability in the provision of services, activities, programs, or benefits by MHMR Tarrant. MHMR Tarrant’s Human Resource Policies govern employment related complaints of disability discrimination.

The following Grievance Procedure may be used by anyone who wishes to file a grievance concerning access to MHMR facilities, services, activities, and programs. The complaint should be submitted by the grievant and/or his/her designee as soon as possible but no later than 60 calendar days after the alleged violation. A complaint can be filed online using the link below, by mail, or by facsimile.

Upon receipt of a Formal ADA Grievance, the ADA Coordinator will review and work to provide possible resolutions.

Keep a copy of your complaint and the original documents for your own records.

Privacy Act Statement
The Americans with Disabilities Act of 1990, 42 U.S.C. §§ 12131-12134, and Section 504 of the Rehabilitation Act of 1973, 29 U.S.C. § 794, authorize the solicitation of the information for this form.  Providing the information is voluntary, except that failure to provide such information may result in the MHMR Risk and Safety being unable to process your complaint. The principal purpose of collecting information from you is for Risk and Safety authorized civil rights compliance and enforcement activities. Risk and Safety will not disclose your name or other identifying information about you unless it is necessary for enforcement activities against an entity alleged to have violated federal law, required to be disclosed under the Freedom of Information Act, 5 U.S.C. § 552, disclosure is permitted pursuant to the Privacy Act, or is otherwise required by law.
