Where Do I Start? 

Step One: For MHMR services, come into any of our Centers or contact us for a phone interview.

• The ICARE Call Center operates 24/7/365 and can be reached at (817) 335-3022. 

Step Two: Visit the closest Center to you and bring required documents:

Required Documents:

• Proof of Residence
• Proof of Income
• Proof of Dependents
• Proof of Insurance
• Picture ID

Step Three: In-person screening completed at any of our Centers.

Step Four: If you are eligible, an appointment will be offered at your Center. If you are not eligible, other community resources will be offered.
